The output is digital, but the thought process is the same as traditional drawing. To draw lines, the window procedure can retrieve a display device context and draw a line in the window between the current and previous cursor positions. I used to draw my digital works solely with a mouse after discovering the stabilizer function in gimp, using a lot of arm motion compared to wrist motion to get long lines smooth.
With the mouse, you'll find it hard to make perfect strokes… there's a way out though in order to be good at drawing with a computer mouse, your hand has to be good at drawing edges and curves.
